Artlab Maui – Liability Waiver, Medical Authorization & Media Release

At Artlab Maui, we’re passionate about creating a safe, supportive, and inspiring space where creativity can flourish. By enrolling your child in any Artlab Maui class, workshop, or program, you acknowledge that you have read, understood, and agree to the terms outlined in this waiver, and you provide your consent to all policies stated herein.

 

Assumption of Risk & Liability Release

I, parent or legal guardian, understand that participation in Artlab Maui classes and programs involves inherent risks, including the use of art materials, tools, and studio equipment. On behalf of my child, I voluntarily assume all risks associated with participation. I hereby release and hold harmless Artlab Maui, its owners, instructors, employees, contractors, and volunteers from any and all liability, claims, or causes of action arising from injury, illness, loss, or damage occurring during participation in any Artlab Maui program or while under staff supervision. I agree to indemnify and hold harmless Artlab Maui from any claims or expenses (including legal fees) resulting from my child’s participation.

 

Medical Authorization

In the event of illness or injury, every effort will be made to contact me. If I cannot be reached, I authorize Artlab Maui staff to seek emergency medical care, including contacting 911, and authorize licensed medical providers to administer treatment as deemed necessary. I accept full responsibility for any medical expenses incurred.

 

Materials & Allergy Disclosure

Artlab Maui uses a variety of art materials, which may include paints, inks, adhesives, pastels, and mixed media supplies. Please advise of any allergies, sensitivities, or medical conditions we should be aware of: While we take care to use age-appropriate and non-toxic materials whenever possible, we cannot guarantee an allergen-free environment.

 

Supervision & Studio Policies
Students are supervised only during registered program hours. Please arrive at the designated start time and pick up promptly. Students are expected to treat others, materials, and the studio with respect. Artlab Maui reserves the right to dismiss any student whose behavior is disruptive or unsafe.

 

Photo & Media Release

We love sharing the creativity that happens in the studio. I grant permission for my child to be photographed or recorded during Artlab Maui programs for use in promotional materials, including social media and website content. No names or identifying information will ever be used without additional consent.
